Emergency Contact Information Card

The Maryland Motor Vehicle Administration (MVA) as a courtesy, provides a wallet sized Emergency Contact Information Card to list emergency contact information. In the event of an emergency, a contact card provides law enforcement and medical personnel information to quickly contact your family and/or designated contact. The MVA encourages you to take a minute to fill out the card. You may laminate the card if you wish for added durability. Periodically, review and update your contact information.
