At 2042, Co's. 4, 14, 49, St. 39 and the medic unit were alerted to the area of Three Notch Road and Bay Forest Rd for a reported motor vehicle accident with one unconscious. Within minutes of the dispatch, A496 (EMT/IVT Turner), LT. 49A (Morris) and LT. 49D (Lowmiller) were en route to the scene.

At 2049, Command 4 and Lt. 49A & Lt. 49D were on the scene with a full-sized pickup truck off the roadway with one laying in the roadway. Both lieutenants immediately went to work triaging the four patients on the scene and treating the female patient lying in the roadway. After a quick assessment, a category Alpha, Priority Two (later upgraded to one) flyout was requested. All helicopters were down due to prevailing fog in the area. SQ4, E141 and A397 were next arriving, assisting with patient care with A397 assuming care of the patient. A496 assumed care for the other three patients and obtained signed refusals.
